  6. A statement that the information in the notice is true and, under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly being violated.

Under Section 512(f) of the Copyright Act, anybody who intentionally and materially misrepresents that content or behavior infringes may be liable.
